If you plan to use private health insurance for your physiotherapy, there are two ways to access treatment depending on your provider and policy.
Steph Lyons Physio is registered with the following insurance providers for direct invoicing:
- AXA Health
- Vitality
- Aviva
- Allianz Partners
- WPA
With these providers, Steph Lyons Physio can invoice your insurer directly for your appointments.
To use this option:
- Ensure your insurer is listed above.
- You will need to obtain pre-authorisation from your insurer before your appointment.
- You will need to provide your authorisation code or claim reference number.
- Ensure you have sufficient sessions approved for treatment.
- Complete the insurance form - this is required to confirm your physiotherapy appointment(s).
How to book an appointment:
1. Check availability and request an appointment
View appointment availability and get in touch with your preferred day and time via the contact form or email:
Please allow enough time before your appointment to contact your insurer for pre-authorisation and complete the insurance form. This information is required at least 24 hours prior to your appointment.
2. Complete the insurance form
Your appointment cannot be confirmed until this has been completed. You can access the form here.
If your insurer is not listed above, or your policy does not allow direct invoicing, you can still attend appointments on a self-pay basis and you may be able to claim the cost back from your insurer (please see 'Important Information' below).
How this works:
- You pay for your appointment at the time of booking via the online booking platform.
- You will be provided with an invoice following your appointment.
- This invoice can be submitted to your insurer for reimbursement.
How to book:
Book your appointment via the online booking platform. Payment is taken at the time of booking.
An invoice will be provided following your appointment, including any additional information required by your insurer for reimbursement.
Important information:
Please check with your insurer in advance:
- That your policy allows reimbursement.
- How much of your treatment is covered.
- Whether you have an excess or session limit.
If your insurer does not cover your appointment for any reason (including insufficient allowance or excess), the full cost remains your responsibility.
